SEVERE NECROTIZING FASCIITIS COMPLICATING APPENDECTOMY

Abstract:

Necrotizing fasciitis is a severe and rapidly progressive soft tissue infection, involving deep fascia and extending to skin, subcutaneous tissue, and superficial fascia. It is a rare but life-threatening complication in the postoperative patient.

We report a case of necrotizing fasciitis of the thoraco-abdominal wall in a 22-year-old woman following appendectomy. Methicillin-resistant Staphylococcus Aureus and Escherichia coli were isolated Intravenous broad-spectrum antibiotic therapy was promptly instituted. Concurrent aggressive surgical debridement of all necrotic areas was required.

ost debridement therapy required a long period of dressing changes. Skin grafting was finally used to cover the huge soft tissue defect. Postoperative necrotizing fasciitis should be recognized as a first-degree surgical emergency.

Early diagnosis, intensive surgical intervention, antibiotic treatment and intensive medical care are crucial for a successful outcome in patients with septic shock and extensive post appendectomy necrotizing fasciitis.
