CHOLEDOCAL CYST – CONSIDERATIONS ABOUT TWO CASES

Abstract:

Adult choledochal cysts are rare and are often non-specific in their clinical presentation. They are classified into six types, all of which are relatively rare. We report two cases of a choledochal cyst at (two women with age 43 and 53 years) whith type I Todani choledochal cysts. This usual variant of choledochal cyst (Todani I) was explored by ultrasonography, magnetic resonance cholangio-pancreatography and intraoperative cholangiography to evaluate the full extent and type of choledochal cyst. We performed complete cyst resection, cholecystectomy and Roux-en-Y hepaticojejunostomy in one case and hepaticoduodenostomy in another case. Postoperative course was uneventful. The epidemiology, diagnosis, surgical treatment, and risk of cancer in choledochal cysts were revised. Primary cyst excision and biliary Roux-en-Y reconstruction is the treatment of choice. Regular long-term review of these patients is mandatory in the surveillance of cholangitis and the risk of possible long-term malignance of this entity.
