Abstract:

Intraperitoneal (IP) administration is an effective route for drug delivery, with a pharmacologic advantage that can be exploited for the treatment of ovarian cancer. There is now a growing body of clinical data demonstrating a survival advantage for patients with advanced stages of ovarian cancer treated by IP chemotherapy compared with the standard intravenous (IV) treatment route. While IP chemotherapy has been shown to determine an improvement in survival, many investigators argue that the formidable toxicity, complexity, and quality of life alterations associated with IP therapy make it mandatory that it should be compared prospectively to a less toxic and more convenient regimen, such as IV carboplatin/paclitaxel. The barriers to implementing this treatment into clinical practice appear to be toxicity concerns, and a lack of technical expertise with the peritoneal infusion devices. The role of IP chemotherapy in colo-rectal carcinoma is not well established. However, cytoreductive surgery and intraperitoneal hypertermic chemotherapy may be of potential benefit in selected patients with locally disseminated disease.
