PROSTHETIC MESH REPAIR OF THE ABDOMINAL INCISIONAL HERNIAS; LONG-TERM RESULTS

Abstract:

The use of synthetic meshes in surgical repair of abdominal incisional hernias became a routine surgical technique in the last years. Analyzing the results, the authors, try to describe the surgical manner to insert the mesh which can prevent the recurrence and the reject of the mesh. In the First Surgical Clinic Iasi (Romania) 1031 abdominal incisional hernias were operated between 1994 and 2003. At 323 patients a synthetic mesh were used. The different surgical manners to insert the mesh are analyzed, with the particularities of postoperative evolution, the recurrence rate and the reject rate of the mesh. In 172 cases (53.3 %) the mesh was placed inside the peritoneal cavity and the postoperative evolution was free of complications. We consider this technique the gold standard but it is dependent by the presence of the great omentum and by the accuracy of the surgical technique.
