Abstract:

As for any intraabdominal neoplasms, the adequate management of colorectal neoplasm mainly depends on the accurate preoperative staging of the disease. Despite significant technical improvements, preoperative imaging studies often fail to predict the presence of peritoneal carcinomatosis, lymph node metastases, occult liver metastases or local spread and resectability of the tumor, which leads to many unnecessary laparotomies with its related morbidity and consequently delaying the start of palliative or neoadjuvant therapeutic protocols. These problems could be avoided by using diagnostic laparoscopy in association with laparoscopic ultrasound, which is more accurate in detecting the intraabdominal disseminated tumor spread as well as local tumor extent. The aim of this review is to highlight the importance of diagnostic laparoscopy in the management of colorectal neoplasm and to discuss about its historic data, terminology, indication and contraindications, advantages and disadvantages, technique, incidents and accidents and possible complications of this very useful diagnostic method.
