TROCAR INJURIES IN LAPAROSCOPIC SURGERY

Abstract:
The aim of this study was to analyze the risk factors associated with
trocars and Veress needle injuries, resulting from their use, in laparoscopic
surgery.
We analyzed 6367 laparoscopic operations, performed in First Surgical
Clinic from 1993 through march 2003. There was 184 trocar injuries:
26 abdominal wall hematomas, 7 bowel injuries and 151 other visceral
injuries. In our records it wasn’t injuries of the major blood
vessels. The injury of duodenum was delayed in one case and it was
necessary a new surgical intervention (classic) in the first 24 hours.
We used different type of trocars: conventional cutting trocars,
with safety shield and in a few cases direct view trocars. We also
made the pneumoperitoneum with Veress needle and when the patients
had abdominal surgical history we used “safe techniques”(open
technique Hasson).
Conclusions: These data show that safety shield trocars cannot
prevent serious injuries. Visceral injuries should be avoidable by
following “safe” techniques.
